ENTRANCE REQUIREMENTS

All children aged 2 years and 10 months old and up are welcome to enroll.

Interested applicants are required to:

  1. Upon application:

a. Pay the application fee:

Preschool—P150

Grades 1 to 10—P300

b. Submit the following:

i. a properly filled-out Student’s Application Form (Click HERE.)


ii. a photocopy of the child’s birth certificate (NSO/PSA)
iii. a medical certificate from the child’s pediatrician
iv. a written evaluation and recommendation from the child’s developmental pediatrician (for children with Special Needs)
v. recent 2×2 photo (2 pcs)

ADDITIONAL REQUIREMENTS FOR INCOMING GRADES 1 TO 10 ONLY:

vi. Original/Certified true copy of the most recent ECCD checklist from the previous school (INCOMING GRADE 1 ONLY)

vii. Original/Certified true copy of Kindergarten completion certificate (INCOMING GRADE 1 ONLY)

viii. Original/Certified true copy of the most recent report card from the previous school
ix. a properly filled-out Student’s Recommendation Form (provided by DUYAN, to be accomplished by previous school’s Guidance Counselor or Principal) (Click HERE.)

c. Undergo DUYAN’s assessment and evaluation.

i. The schedule will be announced later
ii. The results will be announced within 2 weeks after the placement tests and evaluation.

2. Early Registration

a. A student who successfully completes the whole application process is eligible for early registration or slot reservation.
b. Submit the following:
i. a properly filled-out Early Registration Form. (to be requested through admissions.duyan@gmail.com)

c. Payment of the miscellaneous and other fees may be accomplished at this time or during the regular enrollment period.

3. Upon enrollment:

a. Only returning students and new students who have successfully completed the assessment evaluation may enroll in DUYAN.
b. Submit the following:

i. a properly filled-out Registration Form
ii. DepEd Form 137/School Form 10 (requested from school to school and  applicable only to incoming Grades 1 to 10 students) – without which the child is only considered as temporarily enrolled.

4. Pay the registration, miscellaneous, and other fees upon enrollment.
